The Regional Transition Council(RTC) that is facilitated by the Region 6 State Support Team is creating a Transition Resource Directory. The RTC would like to have a resource that educators, parents, students, and professionals can use to access services and resources to help school age adolescents transition from school to employment, post-secondary education, and independent living.

To help create this directory use the link to Survey Monkey to submit information about an agency or service.
