The Ohio Department of Education and Workforce is seeking input on proposed revisions to required and supplemental special education forms for state and federal requirements by Friday, May 23.

As part of the process, the Department is seeking input from stakeholders across the state on the proposed revisions. Each of the proposed revised forms are available in a side-by-side comparison on the Ohio Required and Optional Forms webpage. Survey links for submitting feedback are located underneath each comparison table; feedback is due by Friday, May 23.
 
The Department is hosting virtual sessions on these proposed revisions for external stakeholder feedback. Attendance at these meetings is optional, and the review of the proposed revisions will be recorded. To register, please visit the Ohio Required and Optional Forms webpage.
